STEPS FOR ONLINE TRANSACTION:

1. Fill out inquiry form
2. We will send out application form, and availability map for you to choose your vault.
3. Send back filled application form & 2 Valid ID's with 3 specimen signatures.
4. Send your payments to Loyola's BDO account.
5. A copy of your official receipt and contract will then be emailed to you.
6. Hard copies will be sent through courier.

INURNMENT Requirements & Procedures
(all requirements must be submitted 2 business days prior to inurnment schedule)
  1. Certificate of Full Payment (Photocopy)
  2. Death Certificate
  3. Certificate of Cremation
  4. Valid ID (with signature & picture) of the Grantee/Owner
  5. Burial Permit from Quezon City Hall
  6. Transfer Permit of Ashes (if cremated outside Quezon City or cremated abroad)
  7. Engraving Request Form

Other documents required
Notarized Deed of Assignment by grantee/owner if assigning the columbary vault to another person
  • If owner is not available, a duly notarized Letter of Authority is required.
  • If owner is outside of the Philippines, a duly notarized Special Power of Attorney is required.
  • If owner is a group/entity, a duly notarized Secretary Certificate  Board Resolution is required.

FYI
  • Any special arrangements must be submitted through a letter of request 5 business days prior
  • Inscription Fee must be paid 5 business days prior to the expected delivery date

  • Reconstruction of Certificate of Full Payment - PHP 800.00
  • Certification Fee - PHP 200.00

RULES & REGULATIONS:
  1. Only 4 names may be etched on the vault cover
  2. Pictures/Memorabilia etc. should not be placed on the vault cover
  3. Plastic flowers & flower vase should not be left
  4. Visiting hours is from 8AM-6PM everyday
  5. No catering inside
  6. Lighting of candles beside the unit is not allowed. There's a designated candle bay for floating candles only.
  7. Pets are not allowed
